We look forward to seeing you on your scheduled appointment date. If you are a new patient, information is being sent to you in an effort to streamline the check-in process and answer any questions you may have regarding your visit. We will be contacting you a few days prior to your appointment to confirm the appointment date and time, pre-register your demographic information and remind you to have a driver available to take you home after the appointment if needed.
